FRIDAY 8th July: AuntieLesleyGroomer and lab results

Cato:

We went to see AuntieLesley and she bathed us :( and gave us liver treats :) and said how silly Tara is :):):)   

We do look nice.  Mum says she may get some pics later, only we are very sleepy now.  Tara is snoring, so she may not bloggify today.  Her sprout is getting sproutier.  And we are nice and cool again now.

We also went to Mrs Vets house and Mum paid for my blood stuff, and got a printout.  Wotever that is.

My blood was all OK, said Mrs Vet, and Mum has looked it up on the computer and sort of agrees.  So that is OK, and I am going to sleep now. :)  I knew Mum would sort it.

 

 

Mum:

The lab results say his T4 is 27.2 mmol/l  with a normal range of something like 16-50 (from memory).  26 or below is seen as the low-normal range.

Apparently most vets screen T4 first, then only look at the other things (TSH and T3 etc) if that is out of wack, when in reality, T4 doesn't always show if there are problems, so is not necessarily indicative of normal function, and different breeds have different levels of 'normal' - but I can't seem to find what IS normal for small breeds such as the Shih Tzu.

I should have done my proper reading before going, in which case I would have asked for a full thyroid screening from the start!

- however, since Cato is not showing any extreme symptoms, and seems both well and happy, just a bit heat/cold sensitive and sleepy, I will keep a close eye on him, and then ask for a full thyroid panel test next year.  The test result of 27.2 will be a useful comparison to see if function has dropped over time. :)

Apparently most dogs don't develop hypothyroidism until 4 or older, so it will be very interesting to see if there is any deterioration in the next year.

Write Pam a note, she could explain the numbers for a Shih Tzu. Sleep well both of you...looking forward to seeing pictures later of your new hair do's

I'm glad the vet visit went fairly well.  I can't offer anything in the way of help with any of the medical stuff.  I'm not versed in it at all.

I'm so glad the grooming was pleasant and that Tara's sprout is growing nicely. I'm so happy that they are both comfy and clean.  I know how great I feel when I'm comfy and clean. ;)

Hypothyroidism is actually fairly rare in Shih Tzu.  The name of the thyroid panel performed on the blood sample is TSH/T3/T4/freeT4. . Low hormone levels in the absence of signs of other diseases are diagnostic of hypothyroidism. Blood levels of T-4 are normally 1.0-4.0 micrograms/deciliter. Normal levels of T-3 are 45-150 nanograms/deciliter and normal levels of Free T-4 are 11-43 picomols/liter. T-4 hovering about one unit and T-3 and Free T-4 levels are low-normal still create suspicion of hypothyroidism if clinical signs are significant. Falsely low thyroid hormone levels can be due to administration of steroids (cortisone) or concurrent systemic disease. A TSH stimulation test can be run if the diagnosis is in doubt. (Anf yes, that's a copy/paste I've had for years, lol.)

Ergo, T-4 isn't going to give you the whole picture  As long as Cato is not symptomatic, I wouldn't worry.  Sometimes if a dog is borderline, a low dose of thyroid is given on a trial basis to see if conditions improve, but I don't think Cato is at that point yet.

We did try thyroid on a friend's borderline Mini Poodle a couple of decades ago after I read that thyroid might be some help for her constant Pseudomonas ear infections.  The vet thought we were crazy but finally caved and guess what?  No more ear infections!  Just goes to show that - as I'm always saying - vets don't necessarily know everything, lol.

Thank you @Luna !

That backs up everything I have read and gives me much more confidence.  Am so glad I asked for the test. :)

Cato has had several ear infections since he was spayed (just over the latest one now), so will add that to my Thyroid awareness too. :)

Will play the waiting game...
